Below are forms you will need to fill out before your interview with Mason Clay. If you have not received these forms, please print out and complete a copy prior to your interview.
Please be aware that Mason Clay will also carry out a Disclosure Application Form with the Criminal Records Bureau if you do not have a current CRB. A copy will be sent to your home address and we will keep the reference number on file and shred the original document. We will also take out three professional references from previous employment.
You will also need to bring with you:
- Proof of ID (Birth Certificate & Passport)
- Proof of change of name if applicable (marriage/divorce certificate)
- Proof of Address (copy of household bill or driving License)
- Original qualifications certificates
- Current CV
- 2 passport photos
- CRB Certificate
- Any other relevant documentation.
After placement with us, you can look forward to monthly support visits. In addition, you can relax in the knowledge that our charge rates are very competitive, although our standards and support levels are very high. We are absolutely transparent with both the Local Authority and our locums, in telling both parties our pay rate and charge rate for the placement.
